designed to offer affordable protection against extended

Homecare and Nursing, Prosthetic Appliances, Durable
Medical Equipment – covers the services of registered
healthcare professionals, surgical bandages and
dressings, the purchase or rental of medically necessary
equipment such as crutches, non-electric wheelchairs and
hospital beds, oxygen and other equipment. All
equipment, supplies and services must be recommended by
the physician and approved by Manulife Financial. This
benefit also covers the costs towards prosthetic
appliances such as artificial limbs, eyes, splints,
casts and breast prostheses following mastectomies.
The
maximum per person, per anniversary year for each of
these 3 categories of benefits separately is according

wanting to lead more independent lives, Lifeline may be
a solution. With the simple press of a button, worn in
the home, the 24-hour monitoring service is alerted that
help is needed. Lifetime maximum of 3 months of service,
not including installation fee

Accidental Dental – covers dental treatment required as
a result of an accidental blow to the head or mouth,
within 90 days following the accident; $2,000 maximum
per person, per anniversary year

Ambulance Services – provides coverage for unlimited
ground transportation and $4,000 maximum for air
ambulance transportation per anniversary year (payable
only after Government Health Insurance Plan maximums
have been reached, if applicable)

Hearing Aid - $400 maximum per 4 consecutive benefit

to take advantage of any of the following services:

Medical Advice and Treatment
Planning – upon diagnosis of serious illness or
injury, a review of the individual’s medical files
may be completed to confirm the initial diagnosis
and recommend the most effective treatment; includes
access to the latest technologies and the opinions
of world-class specialists to reduce potentially
serious complications from misdiagnosis, and help
local treating physicians determine the proper
course of action.

Identification of the Right Care
Provider – a customized search of global database of
over 50,000 world-class specialists and
recommendations for specific medical needs; includes
access to a Personal Advocate
-
Care Management – assistance with
the hospital admittance process and medical
appointments, including co-ordination of necessary
information among medical specialists involved,
monitoring of treatment process to ensure medical
priorities are met, and if travel is required,
assistance with reservations and accommodations.
Vision Care
-
$100 maximum per
person per 2 consecutive benefit years to cover the
costs towards prescription lenses, frames, contact
lenses and laser eye surgery

$30 maximum for
Optometrist fees per person per 2 consecutive
benefit years (payable only after Government Health
Insurance Plan maximum has been reached, if
applicable)
Eyeglasses are covered
against breakage and scratches by a repair guarantee for
a period of 2 benefit years. This warranty applies to
lenses and frames purchased with the Flexcare Vision
benefit.
No deductible is charged
to you if your eyeglasses can be repaired. If beyond
repair, your eyeglasses will be replaced and a $50
deductible charged for eyeglasses originally purchased
for up to $250; a $75 deductible, if the original
purchase price was $251 to $300; and a $100 deductible,
if your eyeglasses originally cost $301, or more.

Eyeglasses are covered
against breakage and scratches by a repair guarantee for
a period of 2 benefit years. This warranty applies to
lenses and frames purchased with the Flexcare Vision
benefit.
No deductible is charged
to you if your eyeglasses can be repaired. If beyond
repair, your eyeglasses will be replaced and a $50
deductible charged for eyeglasses originally purchased
for up to $250; a $75 deductible, if the original
purchase price was $251 to $300; and a $100 deductible,
if your eyeglasses originally cost $301, or more.

Cash
payment to cover personal expenses incurred during
hospitalization. Benefit is payable on the first day if
hospitalization is a result of an accident and on the
4th day if a result of illness/pregnancy

Maximum of $50 per day for covered individuals under 65

Note:
During the first 12 months after the effective date of
coverage, cash payment will not be made for costs
related to any illness diagnosed within a 12-month

Medical Equipment – covers the services of registered
healthcare professionals, surgical bandages and
dressings, the purchase or rental of medically necessary
equipment such as crutches, non-electric wheelchairs and
hospital beds, oxygen and other equipment. All
equipment, supplies and services must be recommended by
the physician and approved by Manulife Financial. This
benefit also covers the costs towards prosthetic
appliances such as artificial limbs, eyes, splints,
casts and breast prostheses following mastectomies.

Accidental Dental – covers dental treatment required as
a result of an accidental blow to the head or mouth,
within 90 days following the accident; $2,000 maximum
per person, per anniversary year

Ambulance Services – provides coverage for unlimited
ground transportation and $4,000 maximum for air
ambulance transportation per anniversary year (payable
only after Government Health Insurance Plan maximums
have been reached, if applicable)
