How to get a free quote :

2. In the List of Plans
select the plan you want to get a quote
for.
There are two Health Plans you can
choose from: FlexCare or FollowMe.
Click the link Get a Quote for the Plan you have chosen.
Select
province Ontario.
3. Fill out the form and click the button <Next>
at the bottom of the page
4.
Get your quotes and check the box against the plan you
want to have: FlexCare (Core Plans +
Add-Ons,
Stand-Alones, FlexCare Select).
Click the button <Next> at the
bottom of the page to get your quote
summary.
How to purchase a policy
1. Click the link above to enter the

2. In the List of Plans
select the plan you want to apply
for.
There are two Health Plans you can
choose from: FlexCare or FollowMe.
Click the link Apply Now for the Plan you have chosen.
Select
province Ontario.
3. Read carefully the Terms of Purchase
and click the small box at the bottom of
the page, if you agree to the Terms of
Purchase. Then click the button <Next>,
if you want to apply.
4. Fill out the
Application online.
You can choose to pay by credit card or
pre-authorized collection plan
(withdrawals from your chequing account) .
The company will withdraw the initial 2
month premiums upon approval of
the Application.
You have to decide how you will pay
subsequent
insurance premiums: monthly, annually,
semi-annually.
5. Submit the
Application.
Plan Number and Identification Number
will be sent to your e-mail address:
make sure that you put a correct e-mail
address in the application.
The company will contact
you if they have questions or will send
you the
contract.
The effective date of this insurance is
the first day of the month following
the approval date.
Read the policy carefully,
and if you don't want to have this
coverage, you
will have at least 10 days after
receiving the policy to cancel it and
all premium paid to the company will be
refunded. There will be a letter with
this information in the package.

group health and dental benefits, you will qualify without

specific needs. |
FlexCare®
Core Plans
Core Plans
let you choose between dental coverage, drug coverage or a
combination of both. With each Core Plan you'll also receive
coverage for registered specialists, travel emergencies, accidental
death and dismemberment and more. Your provincial plan doesn't cover
all this.
If you are 65 years of
age or over, you are likely to have special health priorities. That
is why Flexcare® health plan coverage is adjusted to reflect your
needs.

These coverage options are available without a DrugPlus, DentalPlus
and ComboPlus plan.
So you can select one or more Stand-Alones to create your own health
plan:

Flexcare® Select plan provides Extended Health Care
benefits and Best Doctors® Solutions Services, and
then allows you to select 2 of 4 additional benefit options:
Accidental&Dismemberment,
Hospital Cash,
Travel Coverage, Fracture Benefit.
No medical questionnaire is required for these plans.
All plans
