and Loss of Use Insurance

Loss
of Use Insurance helps when life insurance doesn't. Life
insurance doesn't pay a benefit when an accident leads to injuries
like loss of sight, loss of limbs, loss of hearing, or broken bones.

Insurance pays the full amount of the policy proceeds to the
designated beneficiaries if the Insured dies as a result of drowning
or injuries caused by an accident (not due to an illness). There are
no medical questions or physical examination required to obtain
coverage because this plan covers accidents only.
Amount
of insurance offered
▶ 16 to 69 years
of age - from $25,000 to $500,000 (in $25,000 increments)
▶ 70 to 79
years of age - $25,000
The amount insured is
reduced to $25,000 when the Primary Insured reaches 70 years of age.
Eligibility
The Insured must meet the
following eligibility criteria:
a) He/she must be between
16 an 79 years of age inclusive when the application is signed.
b) He/she may not
confined in a hospital or waiting to be hospitalized
c) He/she may not have or
have had cancer, AIDS or any form of pre-AIDS and must complete the
declarations in the application form.

Exclusions
No benefit are payable if
death resulted directly or indirectly from any of the following
causes:
a) Any accident sustained
by the Insured while participating in a sport for remuneration
or in ay kind of motor vehicle competition, race or speed
contest.
b) Suicide, attempted
suicide or intentional self-injury, regardless of the
Insured's state of mind
c) The Insured's active
participation in a public confrontation, riot, insurrection, war or
act of war (declared or not) or any other warlike act.
d) The Insured's
operation of motor vehicle or a boat with an alcohol level exceeding
80 mg per 100 ml of blood or under influence of any drug.
e) The Insured's
participation in a flight or a flight attempt in any aircraft
in any capacity other than that of a passenger.
f) Voluntary or
involuntary inhalation of gas or ingestion of poison or drugs.

This plan provides
compensation in the case of accidental loss of sight, hearing,
speech or loss of limbs.

End of
Coverage
This coverage ends on the
contract anniversary coinciding or following the Insured's 70th
birthday.
